Who We Are

Established in 2005, HealthFinders Collaborative (HFC) is a community organization providing quality healthcare, advocacy, and wellness education for people with limited healthcare alternatives. HFC works across the continuum of wellness to collaborate and engage communities in their own health. HFC believes in the inherent capacity of our patients and communities to support health and wellness.

Our facilities are staffed by medical/dental providers, nurses, receptionists, interpreters, and clinic coordinators.

Services include:

Medical Care: preventive, acute, and chronic primary care are offered at our locations in Faribault and Northfield.

Dental Care: Exams, cleanings, fillings, extractions, and individualized oral healthcare are offered at our Faribault location

Mental Health: We offer clinical and preventive intervention services centered on working to both alleviate psychological distress and produce resiliency and ongoing wellness. Bilingual services are available.

Patient Advocacy: Advocates provide enrollment assistance for MNsure in addition to connecting patients to community resources and social service programs in Rice County.
